Have you received an email with the Subject Line, “Equipment: (BC#XXXXX – Equipment Description) has Failed Electrical Inspection“?
Does your equipment have a red “AHJ FAIL” label?
Does your equipment have a yellow “AHJ CONDITIONAL ACCEPTANCE” label?
If you have had equipment inspected and it either has “Conditional Acceptance” or has “Failed” the inspection, and you need assistance having the equipment repaired by a QEW, you have the following options:
- If you have equipment that needs repair, request QEW Support from the Engineering Division. Support may be requested by completing a request form, via email to qewhelp@lbl.gov, or by calling x6222.
- If you have a Facilities-related repair need, contact the Facilities Work Request Center. A work request can be submitted online through the linked page, or by calling x6274.
- If you need additional assistance, contact your Division Safety Coordinator (third link under Programs/Liasons/Coordinators), or the EHS Electrical Safety Group.
